offsite permission

We like to go offsite…winter camp, disciple now, student leadership retreat…great stories to tell. Incredible life-impacting experiences. Anytime we leave the church property students must have parental permission.
Student/Parent Contract  If attending an overnight trip, we require a signed “Student/Parent Contract.”

      Please turn in this form to Jen’s [JH] or Daniel/Matt’s [HS] mailbox next to their office.   (We prefer that this contract is signed at the beginning of each school year.)

Online Forms  [Preferred Method]

1) PARENTS: Search for the corresponding event below. Click on the corresponding activity to open the online permission form.
[We would really appreciate if you, the PARENT, logged in to your "Connected Bloomingdale Church" account first because: 1) We will know that it is the parent giving permission 2) The permission form will be linked to you in CBC.

2) If there is an activity cost please place the money in a sealed envelope with a “Trip Tix“ in Jen’s [JH] or Daniel/Matt’s [HS] mailbox next to their office

Cash or Checks are accepted payment. (Checks are payable to: “Boomingdale Church Youth Group”).

3) If attending the Winter Camp or Fall Student Leadership trip, each student must turn in: Expeditions Unlimited Camp Waiver + Health Form to Jen’s [JH] or Daniel/Matt’s [HS] mailbox next to their office.
